Convergence Integrated Care (CIC) is an Integrated Health Partnership (IHP) of community mental health and behavioral health programs and providers across Minnesota.

 About Convergence Integrated Care

Convergence Integrated Care (CIC) is a statewide clinically integrated network of 21 independent non-profit community behavioral health agencies across Minnesota. Member agencies formed this network to create a system of community behavioral health care.

CIC provides high quality care and a consistent high-quality experience of care across the full network population - individual clients, families, communities - through coordinated care pathways, improving access to services and treatment and closing referral loops for clients. CIC brings a local network of providers with expertise, knowledge, and relationships with local and state communities.

CIC’s staff team provides member agencies with services to help them build integrated service models, improve quality and outcomes for their clients. The network assists member agencies in designing and improving their work in value-based care and new innovations in service delivery.

CIC Goals

Our goals are to advocate for, build an integrated system that prioritizes and meet our clients’ needs where they are. We are also striving to give an identity to work we are doing, helping redefine community behavioral health care. We are building network-level standards of care across community behavioral health agencies, decreasing variation in service quality and client experience across network providers.

CIC accomplishes our goals by building our data interpretation and decision-making capacity - to collect, analyze and report population health data - to support providers’ delivery of care and treatment for our clients.
