A special conference rate of $84 per night, single or double occupancy, is available at the Holiday Inn Hotel & Suites Downtown Waterfront. All lodging reservations and requests are being handled directly by the Holiday Inn Hotel & Suites. The deadline for lodging reservations is September 6, 2011. Optional Thursday Evening Networking & Social Activity:
Join us on Thursday evening for great networking and the beautiful views of the North Shore Scenic Railroad. We will begin our evening with an excellent dinner buffet at the Radisson Hotel, just a short walk from Duluth's Historic Union Depot. We will depart on a scenic train ride through downtown Duluth, Canal Park, along the shore of the Lake Superior and deep into the majestic North woods. The train ride will feature a wonderful dessert and coffee selection and excellent networking opportunities. Organizations may request reserved seating together for team-building. Cash bar is available. Tickets for $30 must be purchased in advance. Space is limited. Please indicate that you would like to purchase a ticket(s) for this optional event by filling in the desired quantity on the registration form below or on the online conference registration system.

Extra Meals:
Extra meal tickets are available on an a la cart basis for special guests or spouses who wish to dine with the conference participants for only select meals. Meal tickets must be purchased in advance. You will receive the tickets when you check in for the conference. Extra meal tickets may be purchased by using the online registration system, the printable conference registration form, or by calling the association office at (651) 642-1903.

Special Needs:
If you have special needs while attending our conference (i.e. guide, mobility, reader, sign language interpreter) please attach a letter outlining those needs and submit it along with your registration form or email to registration@macmhp.org. If you have special hotel needs, contact the Holiday Inn Hotel & Suites.

Dietary Requests:
If you have special dietary requests (i.e. food allergies, gluten Intolerant, kosher or vegan meals) for conference meals, please attach a letter outlining those needs and submit it along with your registration form or email to registration@macmhp.org. Send special requests ASAP and no later than September 10th.
